ACCEPTABLE ID FOR ISSUANCE OF BIRTH CERTIFICATE

 PRIMARY DOCUMENTATION (1 REQUIRED)

  • Valid driver’s license (in state or out of state)

  • Military ID

  • State issued ID card (in state or out of state)

  • Valid Passport

  • Department of Correction ID issued in the past 6 months

  • School ID with signature & photo

  • Court order(must order health dept. to release record to person named on record)

  • Current Car Registration if applicant’s name and signature on doc.

 SECONDARY DOCUMENTATION (2 REQUIRED)

  • Police/Fire report (if ID has been stolen or destroyed by fire)

  • Employment ID with signature, photo, date of employment or employment address

  • Bankcard with signature (NOT credit cards or personal checks)

  • Voter registration card

  • Pervious year’s tax return with signature & SS# (NOT e-file)

  • WIC, Food stamp/welfare ID card

  • Probation documents or statement from Probation Officer on letterhead stating individual’s name and date of birth (DOB)

  • Letter from BMV or SS stating individual’s name and DOB

  • Certified copy of marriage license application showing name, DOB, parents & signature

  • Expired driver’s license if expired within last 2 years

  • Gun permit with signature

  • Social Security Card
